If a truly local warung has ice, be assured that it will be safe. Locals can't afford to get sick, and a warung not catering to tourists needs to cater for its repeat customers and serve clean food and drinks.
Salads are another thing. Indonesians don't eat salad and if you served a salad they would think it was because you can't cook. Therefore their food prep is based on any bugs being cooked out. Avoid salad except in good western restaurants.
