I never worry about ice, but avoid buffet food like salads and other meals sitting to cool/warm for hours.

Eating cooked meals seems safer to me, but the big thing people forget is the need to clean your hands before you eat. No matter how careful the staff are in a restaurant if you then pick up food, or pick your teeth or whatever, with fingers that have touched a myriad of surfaces for hours beforehand then you are exposing yourself to germs galore.

Since my family has made a point of washing hands when we can, and always using sterilising gel before eating food, it is rare for any of us to get tummy troubles in Bali.
