in Indonesia in my opinion and in fact over much of SE Asia. Just take a look at what happens on building sites etc etc. Never forget that many local people also die from drinking "market arak". It seems to have crossed over into the tourist bars since the increase in tax on imported alcohol. It is noteworthy that the only time it really gets much attention in Aus is when (horribly) Australians are killed or maimed.
