You pay for the vip bit with netfare and on arrival someone from their company will be standing there with a sign with your name on it.
They will take you to a comfortable room and take your passports and give them yor visa money whether it be 10 or 25 us . They will do all the paperwork etc and collect your bag.
When all is done they will walk you out into the exit area of the airport..
Below is the email I got back from Netfare...

Our VIP VOA Bali service allows you to skip the long queue of Immigration and have everything sorted out while you wait in a lounge, having a drink or some snacks. It really is a VIP treatment.
Our services costs $38 (AUD) per person, this cost does not include the VOA fee of $25 USD that is payable in Bali.
With the service, a Concierge in Bali will wait for you when you get off the plane with a sign holding up your name. He will then ask for your visa money, passports and luggage tags. After he obtains these things from you, he will take you to the lounge and you will wait at the lounge while the concierge takes care of everything. Once it is all complete, he will take you past immigration and outside the airport and towards your preferred method of transfer to the hotel!
