From most of the major airports it's Rp. 75.000, from some smaller airports it's less.
Citilink and Garuda have included the domestic PSC for some time now, other airlines are expected to do the same shortly (if it hasn't already happened).
I booked return tickets with Garuda from Perth in October last year and Citilink from Denpasar to Surabaya with Citilink at the same time. (Just arrived back in Perth last night).
On the Citilink flight we had a separate queue to go through because the PSC was already included in the ticket price, all other domestic travelers had to cough up their 75.000.
When we departed Denpasar for Perth we paid Rp. 200.000 each at checkin and they put a big stamp on your boarding pass that says "PSC PAID". They do give you the option of paying later on through the immigration process if you don't have enough cash on you at the time. There are ATM's after checkin.
Signs everywhere saying that all international tickets booked from 1st Feb onwards will have the PSC included as others have stated.
