Last time I flew in to DPS AA had "run out" of the arriving passenger cards and I had to fill in once through the VOA desk.
From memory they were just photocopies.
Having a look online it seems there is a barcode on the form but obviously if they are just photocopying them a unique number is not required.
If someone can have a look next time they arrive and report back it may pay people to print some of the web and have them pre-filled.
