If you don't trust agents in Bali then get a 60 day visa in the States. Really, I do think you are being quite silly. My husband gets a 60 day visa from the Indonesian Consulate here in Perth every year &, believe it or not, his passport is returned a week later intact with the visa. He has also often used agents in Indonesia & so far his identity has not been compromised - wouldn't mind if a Brad Pitt look alike decided to be my hubby!
Stop worrying. You will have a vile trip if you are so angsty already. Relax.
