Taking refrigerated medication

I now have to take 2 lots of medication that requires refrigeration.

Because I am catching 3 planes to get there this trip I wonder if others have taken it in hand luggage and are you allowed to take a cloth esky with ice packs. From leaving home I will be 17 hours before I get to the fridge of the hotel.

Can I hand over the esky to the flight steward. I will be in transit for 3 hours on 2 legs of the marathon journey.

In the past I have placed in in my cargo luggage so I guess I could just risk it

Any thoughts/suggestions appreciated
