When did you get passports?

Got ours in May 0f 2009. Used them as recently as OCT 2010. We never got a white card. The only white card I know is the Arrival/departure card which they stamp as you leave Indonesia. You then need show this with Indonesian passport on next arrival.
Never got any white card with passport. What does it look like?
