I have hired motorcycles in Bali ten times, but only from fellows working from the footpath.
Usually I do not pay for insurance, as I have never seen any evidence that a policy even exists. Often, the hirer will suddenly ask for money for insurance just as you are about to take off on the motorcycle.
You can ask if the rate quoted includes insurance.
On the Thorntree Forum, I read of a fellow who had a successful experience with insurance. He paid for "Insurance", but then insisted that the English words "Motorcycle fully insured for repair or replacement" be written on the receipt.
Subsequently, he rode the motorcycle into a tree, breaking the frame. Naturally, the hirer was unhappy and demanded restitution. The words on the the receipt were pointed out to him and the rider walked away without paying anything.
If you damage a motorcycle, I suggest that you pay for a bengkel to fix it before returning it to the hirer. Often, the hirer will make ludicrous claims for the cost of repairs.
