too with AA and I did as you did. The failed thing with the credit card was different as I had gone all the way through entered my credit card and got an itinerary up. If I hadn't have checked the booking a bit later I would not have known it was pending payment. I would however had woken up when an itinerary/confirmation did not arrive by email. Yes as you say probably poor quality software.

With Jetstar yesterday it was a little different but I still did not get a message saying "payment failed" etc. it just kicked me out at that point a few times but then when I tried the other credit card it worked fine first time. Still don't know why but all ended well as there were still fares available 2 for 1 on the date I wanted.

Time of course is very often at a premium when trying to grab cheap fares so my advice to anyone who gets as far as entering credit card details, after a few failures to get even that far, then gets kicked out, is to try another card, just in case.
