We took our baby and had no problems, the pram situation can be difficult as you mentioned the footpaths are great, we stayed in Tuban and would use the beach walk to walk up to Kuta, this path is great. In and around the streets of Kuta is difficult and if you are going to guts it I highly recommend you go early before the heat of the day and before the crowds! We went into discovery mall and bought a 3 wheeler pram which was approx $100AUD - not the best pram but did the job, make sure you take a mozzie net for your pram from home if you are going to put baby in there at night when you go out for dinner.
In the cabs we just held her in the back, there is no other option, we met some people who actually "borrowed" a infant seat belt from the aircraft but I personally don't think this would be very safe if in an accident either. We didn't travel long distances so no major drama. Backpack would be great option to take along though.
You can buy formula and nappies etc in the supermarket but the nappies aren't like our huggies at home so suggest you take some of these for nights so you don't have accidents! Also recommend you take baby panadol etc from home as the one we got over there didn't seem to work as well as our panadol. Our little girl got a nasty cough and runny nose while over there and I found it difficult to get something to stop the coughing, so perhaps take something for this as well just incase, especially since you are in and out of air cond and the water best to have it just incase.
We are going back in April/may for a month with our 2 year old and our baby who will be 5 months and we can't wait!
Have an awesome holiday :)
