My daughter and I are Australian citizens. My wife is an Indonesian citizen. My daughter and I queue up in the VOA counter and pay the equivalent of $US25 each. My wife goes to the counter for Indonesian passport holders.

Despite that the customs form says that you only fill out one form for each family, in practice you will need to fill out two forms. One form for the Indonesian citizens and the other form for the Australian citizens.

Indonesian citizens do not always go through Imigrasi quickly. Once my wife was joined in the queue by a kasar Balinese fellow who swore at the Imigrasi. The Imigrasi then staged a "go slow" demanding that all the Indonesians had to fill in the forms again and telling them that no new forms were available.

My daughter and I had to wait for half an hour at the baggage for my wife to escape from Imigrasi.
