Hilary is right. To obtain a 60 Day Visa, you need:
$A60.00 Money Order payable to the Embassy of Indonesia;
Two completed Visa Application Forms;
A valid passport with at least 12 months validity;
One passport photograph;
An itinerary; and
Evidence of a return ticket.
If you go in person to the Indonesian Consulate in Perth, the above will be sufficient. If you post your application, you will need to include a self addressed registered envelope.
