The Balinese within their family surrounds would be more likely to celebrate their 'Balinese' birthday, which runs off the Balinese calendar and is every 210 days, where they'll have a meal, prepare offerings and tuck in some prayers. This is known as their otonan. So you'd probably say "Selamat Otonan"- if off the Gregorian calendar, then "Selamat Ulang Tahun" as previously posted.
