For just a tourist visit no sponsor/letter is required for the first 60 days (as per the old Tourist Visa)
After that you can extend in Bali now, but no, it does not list specifically you need a sponsor for the extension.
However when family members have done this in Bali last year they have used an agent which I was told acts as their sponsor.
They also had to produce passport photos again.
Therefore I am assuming that the extension after the intial 60 days
requires a sponsor.
If anyone has extended without requiring a sponsor it would be great to know to clear it up.
The site does say that if you are applying for a social/family or business visit then a sponsor letter is definitely required upfront.
You also need the sponsor/letter again each time you extend.
eg:
"3. For a business visit, a sponsor letter from the company / organization in Indonesia should be presented.
4. For a social/family visit, enclose a sponsorship or invitation letter from the sponsor or family member and copy of their ID card."
