A different VOA question

Second time trip to Bali, but first since the VOA requirement. I've sussed out obtaining a visa from the Indonesian Embassy here before we go - NZ$60 plus submission of application form, including passport photos etc. What's the procedure for On Arrival processing - do you need to supply photos etc?
