I just rang them while in Bali last October. Pretty sure I rang about 3 or 4 days in advance but we were pretty flexible with when we could go. They were fine with the first day I requested and there were only 7 in our class, including the 3 of us.
It's in Sanur - I couldn't tell you exactly where as I'm not overly familiar with the Sanur area, but they pick you up and drop you back at your hotel. I think we paid an extra few dollars each for Legian area.
They have a website with all the details regarding price and you can probably book online via the site if you prefer, but I had no problem booking over the phone as far as language barriers or anything like that.
