Just a thought....what about if you hired a porta cot from Bali Baby hire and then you would just need to request an extra bed.
Kuta Paradiso, Adhi Jaya and Ramayana all have family rooms. Kuta Beach club allow you to have 2 children in the room.
A bit further south in Tuban there is the Bali Rani, Holiday Inn, Discovery Kartika Plaza, Febris and Dyansty which all have family rooms.
