I think I paid about Rp50K for set up and a small amount of credit, then bought extra credit on top. I know you can get them cheaper at little booths, but I've always had trouble with them not knowing how to set up internet access if you're not using an iPhone, so don't mind paying a couple of dollars more if the person knows what they're doing.
I think I paid Rp50K per month for unlimited internet for a month on the phone, so I could check news, emails, facebook etc.
There's plenty of places around that sell SIMs and credit top ups though. It's just a matter of how good they are at setting up your particular phone. Ask around near where you're staying. I'm sure you'll find somewhere easily enough.
