I use to be a travel agent and I know we use to recommend transfers for first time travellers that weren't feeling confident to arrange their own transport. Although as i have been to bali quite a few times I would definately encourage people to arrange on arrival. It's not actually the travel agent making the money on it as they run on around a 10% commission level so $2.80 is not going to make or beak them but it's more the wholesaler and tour operator in country of arrival.
Yes travel insurance is a quick money maker but it's like any insurance pretty much the same price regardless of where you buy it and if you can't afford it you have to ask yourself if you can really afford to travel.... From memory it's around 30-40% commissionable to agents so not quite 200% :) But like any business if you aren't making money why would you do it...
