I've never met an ATM in Bali that had a Rp400K limit. Most seem to be around the Rp1.5M limit with others allowing Rp3M withdrawal at a time. Most will indicate on the front of the machine what the limit is and what denomination notes will be dispensed (Rp50K or Rp100K).
Also, not every transaction will have a bank fee when you get home. That will depend entirely on your own bank and you should ask them before you leave home what their overseas ATM withdrawal fee is. My credit union charges no fees at all (and gives good exchange rate). You should also notify your bank before leaving home that you intend to use your card in Indonesia - and the dates you will be away - otherwise they may freeze your account on suspicion of fraudulent transactions.
