it just a case of go to immigration at outside the airport fill in the form and hand them in, they will tell you to come back a few days later to pay the 250,000 then they will tell you come back on another day to collect your passport with new visa. if you are staying nearer the denpasar immigration office you can do it there instead of going to airport one.
also its better if you can go 10/14 days before it runs out as they mump and moan if to near expiry date.
