I found 1 Cover to be very cheap travel insurance with all the usual inclusions but I am paying a bit more and going with Defence Health only because by sitting the Balinese motorbike license test I am covered on rental bikes.
As for the VOA (Visa on Arrival), you pay for this at Denpasar on arrival. I usually get $25US at the airport in Australia before departure. You go straight to the VOA counter and pay for this before going through customs.
