At Lombok I go to BNI or other large banks and give my credit card with a copy of my passport (front page) and draw $500. The ATMs there dispense only $125.
My card is issued by my Credit Union. The conversion is at "Inter-bank" rate but these days the banks have started charging additional fees like conversion.
Cash is the best option and if you have access to a safe in your room or a safety deposit box in your hotel, you should be able to handle it.
