Depending on your spend amount (I'll assume 'budget' to 'moderate') I can recommend Tegal Sari or Greenfields (next door) which are both to the south of the Monkey Forest and very nice properties (fantastic rice paddy views) and not too far from the action.
Also, the 'Alam' resorts seem to be quite popular among forumites.
Hotel Tjampuhan is in a lovely 'jungle' setting cascading down a ravine to a river and highly recommended.
Just jump on TripAdvisor and look at their recommendations.

As for restaurants......too many to mention, suffice to say most are pretty good in Ubud.
You're just going to have to take a chance.
I would suggest you pick a hotel first, then look for/ask advice on good restaurants after that.
