When I use an ATM in Bali/Lombok, my bank in USA not only levies a service charge for each use but also a foreign exchange conversion fees. The only way to avoid these is to take $100 bills but then you are dependent on the daily rates that fluctuate and you have to rely on a safe deposit box in your hotel.
There are couple of options I have used to minimize the charges.
1. Even if there is a limit on an ATM (in Lombok it is around US$130, I withdraw couple of times i.e., not take the card out but do a withdrawal again.
2. I can go to a bank with a copy of my passport (first two pages) and my Visa card and they will give cash for upto US$500. My Visa card is issued by my Credit Union and their service charges are little bit cheaper than my bank's.
