Definitely cash in an envelope as a gift to the couple. The amount will depend on how well you know them and how much you wish to give. Any amount will be welcome.
Definitely, at the very least, a sarong and sash with a nice blouse or top. No need to get a kebaya made, unless you wish to. I wore a nice new cotton blouse with a good (borrowed) sarong and sash. Actually, the bride had the sarong and sash for me and even helped me put them on, including a flower over my ear, which was very special for me.
You will have a wonderful time. Don't be afraid to get up and watch what's going on, take pics and video etc. The wedding I went to was in a village in Karangasem and nobody could speak english aside from the bride and groom, my driver (who also knew the groom) and the groom's cousin. Luckily for me the cousin took me under his wing and explained a lot to me, told me to get up as close as I wanted etc. A lot of fun was had by all the ladies asking me questions - with my very limited Indonesian - especially when they found out I was single. I had every single male pointed out to me from that point onward, lol.
