Any place where you see the sign "Pulsa" is a place that sells SIM starter kits and additional credit(Pulsa).
However, many places do not speak English or are amateurish in setting the SIM up.
I normally use the fellow on the right hand side as you come out of the Matahari Department Store in Kuta. Setting up the SIM seems to be complicated, so it is best to get the seller to set it up for you.
