As to the other vaccinations - you can get Hep A in any country in the world ( its not just a third world problem) so I upfdated all mine before I started travelling. Hep B can be combined with Hep A so why not ....( Hep B mainly issue I think if you change sexual partners or blood products you have extra protection)

I work in a hospital and we get travellers all the time with complicatiosn post travel as they decided not to bother with vacinations or precautions ( malaria treatment a little different as its tablets, doesnt agree with everyone, and cant be taken long term I think? again Dr will advise)
