Not "every single card" has fees at all. Just like gillb, my credit union also charges me no fees on overseas ATM withdrawals and never has. I've been using it in Bali since 2003 and I've had no problem with exchange rates either. I check every so often via phone banking (Bali sim card is great for this) what my withdrawals are worth in AUD$ and do a calculation on exchange rate. It's always been within about Rp100-200 of what's the going rate on the street at the time.
