I always use ATMs in Bali and end up with very close to the going rate on the street - generally within Rp50-100, so very close. My credit union charges me no fees at all for using my visa debit card in overseas ATMs.

My dad generally uses the same ATMs but uses a different bank to me. He usually gets a slightly different rate - and gets charged a fee, which is listed as a separate transaction to the withdrawal.

As for the $5 charge that smithers refers to - that charge will depend on your bank too. Some charge more, some charge less. It pays to call your own bank and find out what fees they will charge before you leave home - also to let them know you will be using the card in Bali ATMs so they don't freeze your account.
