You could try Kimberley Spa, just along from the Kuta Beach Club Hotel. They have package rates or individual treatments. Very budget, not a lot of ambience, can get noisy. Depends on the other customers. Seperate cubicles for massage, just room for the table, have double rooms with curtain between if you are with a friend. Some customers are a little loud forgetting that some people are there for quiet massage. Other treatments are done in the front room on lay down recliner type chairs with other customers around you. I've been there when 4 or 5 Koreans have come in for foot massage and they've giggled and joked the whole time through it in their language of course so couldn't join in, other times it's been as quiet as a mouse.Male and Female staff. If your thinking massage everyday then 50,000 rupiah will get you it, try the beach for 30,000rupiah.
Or go a little more upmarket and try Green Garden Beach Resort and Spa. Has full on Bali ambience, great package deals, very clean. They have a pool, sauna/steam room, seperate outdoor cubicles, baskets for your clothes which can be locked away, paper knickers for you, male and female staff.
Once in Bali you will be unindated with flyers advertising spa places as you walk the streets.
