You must have an international drivers license stamped with VALID for the motorbike option (which basically means you have to have a motor bike license to start with).
They have really cracked down on this in Bali, although you can usually bribe your way out of it.
We have done trips with an international drivers license stamped NOT VALID and VALID (motorbike)- we were pulled over 4 times in a 2 week holiday (for no reason other than being tourist ha ha) and talked our way out of it each time paying no money pleading ignorance (in australia you dont need a motor bike license to ride bikes of this CC, we are leaving tonight and just returning the bike etc etc) - its a pain in the butt!!!
Much easier now we have a VALID stamp :-)
Your travel insurance will not cover you for anything if you do not hold a motor bike license in your country
