They have the change sitting there in pre-prepared piles. I hand over my $50 note, they immediately hand back a pile of rupiah. It's perhaps a split second longer than not receiving any change and does not hold up the line at all. It's not like they're fumbling around for change in a cash register like they do at the supermarket.
It'd take me much longer to hunt down a place at home that could supply me with a US$20 and $5 note to pay my exact visa cost - not to mention the crappy exchange rate and petrol spent to get there.
And the change I've received in rupiah has always been very close to what I was expecting - to within about a dollar of what I'd worked out before leaving home.
