As you come out of the Matahari Department Store in Kuta, there is a stall on your right. Ask the pleasant fellow there to set your phone up. He will install a starter kit for you and tell you which prefixes give the cheap calls. It is better to let him set the phone up as it is complicated with Indonesian prompts.
I normally use a Mentari Sim which lasts four months, unless I recharge it beforehand.
