You did not have to pay extra for the performances. You do however have to book in advance if you want a table closest to the performers and I think you have to leave a deposit. Food and Service is all good. I don't know about complimentary transfers, but transport is cheap in Ubud.
There are many performances in Ubud everyday of the week. If you just want to go and watch a show or can't get into the Lotus. The restaurant actually fills up somtimes and you can't get in if you haven't bookked until approx. 10pm after the show has finished. Shows around Ubud go for approx 1.5 hours, then you could go somwhere for dinner. They sell tickets on the street for these perfomances. Just go for a walk and someone will ask if you want to buy a ticket.
