I suggest you go to Kuta first where the plane lands. A good place and time to get to know the place. You have the choice of staying in Kuta or a bit further up the road at Legian or Seminyak. Kuta easily accessible from there. Sanur is quieter and a taxi ride away, you could visit it from Kuta or have a few days there. Ubud is an hour away by taxi/car/bus easily accessed. Lombok - fly there and stay in Senggigi you can see the island easily in a day or so - if you want some bliss and quiet go to Gili Trawangan for a night or two - I would work it the following:
Kuta: 6 days
Lombok: 4 0r 5 days (fly it only takes 20 minutes Indo Airline,Merpati or Garuda)
Sanur: 3 days
Ubud: 3 days
BAck to Kuta for the plane.
30 day Visa On Arrival at Denpasar, Kuta. No visa is required for Lombok.
Locate a copy of the Garuda holiday brochure and a LOnely planet book - full of potential accomodation.
Enjoy - you will have a great time!
