Ubud transport: Personally I would hire car/driver with seat for bub I just feel safer on the longer trips. Around Kuta/Legian I have just travelled with bub on lap obviously not ideal but what most people generally do.
Safari/waterbom: Again personally would probably not bother with Safari Park for 15month old (I have also never been) but waterbom is always a fun day out and a nice way to cool off which bubs (and adults) of all ages will enjoy.
Stroller: Yes def take one or hire one.
Meds: Again take all your childrens panadol, dimetapp from home and anything else that bubs needs...I know you can buy stuff there but I feel more comfortable using the brands that I know. Also take gastrolyte, bub friendly mossie repellent, sunscreen etc
A number for a medical/Dr facility near to where you are staying. Our last trip our 3yo had a nasty fall and required stitches in her forehead (not sure who was more in shock mum or daughter!!) and we were very impressed with attention she received from Padma Clinic in Legian...everything sterile, clean and Dr and Nurses all very good.
Have a great holiday.
