You should ask the person selling the credit (pulsa) for your Balinese SIM to install it. It seems to be an involved process using recorded prompts in Indonesian.
I use the stall outside the entrance the Matahari Department Store in Kuta to buy credit for my Balinese SIM. He set it up for me and advised me of prefixes for cheap phonecalls overseas and the number to ring to establish the amount of remaining credit.
