Have not seen one in Bali. However we, my wife and I went to one on Panang, (Batu Ferringhi) in March of this year. Yes, the water is suppose to be filtered etc, however I did see what appeared to be some fish waste in the water, (plenty of more other dangerous things around). Aside from this, the sensation from the fish nibbling at ones feet, and the resultant laughter, was worth the money that it cost.
If you get the chance try it.
