If you were hospitalised and your travels were upset due to rabid dog bite, motorvehicle crash or any nasty infection etc I am sure you would be covered by travel insurance whilst on holiday. The issue with swine flue may be that many tourists dont find out they have swine flue until they are back home, when the travel insurance cover is passed.
ps i am not an expert on travel insurance, or H1N1 virus.
