I would email him now with as much detail as possible. I understand that he does major stuff himself at his surgery in Denpasar (as opposed to minor treatments like scale & polish which are done by an assistant at Bali Galleria). Your husband might need to make several trips so try for an appointment soon after you arrive. FYI my quote for implants was Rp8million each last June.
