When we travel (regularly) my husband carries an epi-pen. He's never had any problems getting in or out Bali/Australia however we do carry a doctor's letter just in case.
Some hotels have a nurse available at all times(complimentary at Dynasty) otherwise BIMC in Kuta is fantastic. I don't know how much you'd pay, but as far as cleanliness etc its great.
