The suggestions I got 2 years ago were to take a painkiller an hour before my flight and to use ear planes.
The pharmacist where i got the earplanes from said to also take the painkillers an hour before descent. Although ear planes are only meant to be used for one flight, he said using them for two flights is ok.

The instructions on the earplanes packet do suggest before inserting the earplanes pinching your nostrils shut while blowing gently down your nose at the same time.

The earplanes are put in just before the plane begins it's ascent,and removed when the plane stops climbing. On descent they are not removed until the cabin door is opened on landing.
I've followed this advice and for the first time ever have been free from excruciating ear pain. No ear pain 12 flights later! (Makes me sound like a jetseter but it was just two little holidays to Asia with short -stopovers on the way)
