Address is Kuta Square along jl Imam Bonjol on the left heading towards Denpasar, There is a huge bus depot there and most taxi drivers know where it is. Not far from ACE Hardware, if u drive out of jl Sunset heading towards Kuta you almost run into it. If u r looking for something upmarket and different to anything you have seen in Bali before you will have found the right place. The dress maker Belinda will finish it for u that is for sure if u want to email I can give you their number as I will have it somewhere.
