The fee is $US25, so the rupiah price depends on the exchange rate on the day you go. I can't see much point in buying from street vendors, it's easy enough to pay when you get there, and from what I saw from other people at our hotel, they didn't save anything by pre-buying.
To arrange the courtesy bus, I'm sure your hotel will call and do this for you. We asked the guy from the tour desk at our hotel how much entry was and he just called them and asked and told us, so I doubt it'd be a hassle to arrange the bus for you. We ended up using our own transport though, just for the freedom of coming and going when we wished to. Also because we didn't fancy traveling to Kuta to get on the bus, since it has only one pick up point.
