I have on many occasions used the BCA Card Centre on Jl Raya Kuta for amounts of up to Rp10,000,000 per transaction with a local fee of Rp30,000.
Your bank charges will vary depending upon which financial institution you use.
Photo ID ( passport or international drivers licence OK ) is necessary.
The BCA Card Centre is on the same side of Jl Raya Kuta as Bali Bakery heading towards Kuta.
