Two weeks ago in Bali I went to Dr Rudy Salindeho's surgery in Kuta Galleria. A lovely competent lady cleaned my teeth (for Rp400K) & quoted for implants - Rp8million/tooth - only done at the surgery in Denpasar. Don't know about teeth whitening but if you do a search here you'll find lots of info.
You can email him at rudysalindeho@yahoo.com He replied promptly to my emails while I was in Bali. (I went to the Kuta Galleria surgery but it's only open between noon & 8 pm). I don't think he has a website.
