You'll be able to get VoA in Surabaya. If you fly Garuda through to Surabaya, you will need to clear customs and immigration in Denpasar(where you would get your VoA), and the Denpasar-Surabaya leg will be a domestic flight. If you are flying Malaysian or Silk Air you will be able to get a VoA in Surabaya upon arrival.
